Welcome to Hammond Health’s website. By accessing or using this site and the services provided, you agree to the following terms and conditions. Please read them carefully.

1. Services Provided

Hammond Health is a 100% virtual psychiatric-mental health practice led by Jilian Hammond, PMHNP. Services include medication management and psychotherapy offered via secure telehealth platforms. Please note, I only see patients located in Illinois.

2. No Emergency Services

Hammond Health does not provide emergency, crisis, or urgent psychiatric care. If you are experiencing a mental health emergency, please call 911 or go to your nearest emergency room immediately.

3. Appointment Policies

New patient intake appointments are 60 minutes.
Follow-up sessions are 30 minutes.
Cancellations must be made at least 24 hours in advance to avoid a $100 cancellation fee.
Payment is due at the time of service unless otherwise arranged.

4. Telehealth Disclaimer

Telehealth services are provided in compliance with applicable laws and regulations. Due to the nature of telehealth, there may be risks including interruptions in service or technical difficulties. By using telehealth, you acknowledge and accept these risks.

5. Confidentiality and Privacy

All client information is kept confidential according to HIPAA regulations. Please review the Privacy Policy for detailed information on how your data is protected.

6. Payment and Insurance

My billing company is Headway, which processes insurance claims for clients with coverage. If you do not have insurance, I do accept self-pay clients. Payment policies, including copays and fees, will be discussed during your intake.

7. Website Use

The content on this website is for informational purposes only and does not replace professional medical advice, diagnosis, or treatment. Use of this site does not create a provider-client relationship.

8. Limitation of Liability

Hammond Health and Jilian Hammond are not liable for any damages arising from the use of this website or telehealth services.
